How to Apply for a Christmas Basket

ELIGIBILITY:
Residents of zip codes 95815, 95833, 95834, 95835, & 95838
 
SIGN-UPS:
  • An adult who lives in the house must come in person with all the proper documentation for everyone living in the house to apply for the basket
  • Applications will be taken beginning Tuesday, October 23, 2018 from 9:00-12:00 noon.
  • See calendar on back for additional details on dates and times for sign-ups.
 
DOCUMENTATION:
  • The adult making the application must bring photo ID
  • For each adult (18+) in the household, bring two CURRENT (October/November/December 2018) items with name & address, such as utility bills, phone bills, letters with legible postmark, etc/
  • For each child ages 5-17 years in the household, bring a copy of the 2018 school registration form with name, address, and date of birth.
  • For each child 4 years & under in the household, bring a copy of their birth certificate.
Please note:  photo ID’s from schools, vaccination records, social security cards, and Medi-Cal cards do not have addresses and cannot be used for proof of address for children ages 5-17.
 
GENERAL INFORMATION:
  • Only those who can prove that they live in the household can be put on the application
  • We are unable to add people after the application has been completed
  • Only one basket per household/address will be provided....families who are sharing housing will receive one basket based on the number of people living in the house
  • At the time of registration, families will be given an appointment day and time in December to pick up their baskets.
